Who qualifies?
You're signed up for VA health care, and we conclude that you need a specific service to help with your ongoing treatment and personal care, and the service (or space in the care setting) is available near you.

Community HealthChoices (CHC) is a new mandatory Medicaid managed care program. It is for people who qualify for both Medicare and Medicaid and individuals with physical disabilities. CHC uses health plans to coordinate physical health care and long-term services and supports (LTSS). A health plan will give you better access to health care.
Who qualifies?
CHC is a mandatory program for people who are age 21 or older and qualify for: Both Medicare and Medicaid; or Medicaid LTSS because you need the level of care provided in a nursing facility or through one of the home and community-based waivers.
The OBRA Waiver is a Home and Community Based Waiver program that may help you if you have a developmental physical disability to allow you to live in the community and remain as independent as possible.
Who qualifies?
Age 18-59 individuals who are currently in the waiver prior to 7/1/2006 will age in place. Have a severe developmental physical disability requiring an Intermediate Care Facility/Other Related Conditions (ICF/ORC) level of care. The disability must result in substantial functional limitations in three or more of the following major life activities: Self-care, communication, learning, mobility, self-direction and capacity for independent living. Meet the financial requirements as determined by your local County Assistance Office.
If you have a physical disability, the Attendant Care Waiver and state funded Act 150 program may be available to you to continue to live in your home and community with support and services.
Who Qualifies?
Be a resident of Pennsylvania Meet the level of care needs for a Skilled Nursing Facility Be between eighteen (18) and fifty-nine (59) years of age Be capable of a) hiring, firing, and supervising attendant care worker(s); b) managing your own financial affairs; and c) managing your own legal affairs For the Medicaid Home and Community Based Waiver Services Attendant Care Program, meet the financial requirements as determined by your local County Assistance Office. Have a medically determinable physical impairment that is expected to last of a continuous period of not less than twelve (12) calendar months or that may result in death To take advantage of the Attendant Care Act 150 Program, you may be assessed a minimal co-payment. This co-payment is based on your income and will not be more than the total costs of services.
